What is a Superbill?
A superbill is a detailed receipt that includes all the information your insurance company may require to process an out-of-network reimbursement for acupuncture. It includes:

Date(s) of service

Fees paid
You can submit this superbill to your insurance company to request partial or full reimbursement, depending on your plan.

What Services Are Eligible?
While Acupuncture is the most commonly reimbursed service through out-of-network insurance benefits, some plans may also cover Manual Therapy or Therapeutic Massage — especially when billed using appropriate medical CPT codes.
At JazHands Acupuncture & Therapeutic Massage, eligible services for superbill inclusion may include:

How to Request a Superbill
When booking as session, mention to the person booking you to add a note for a superbill or if booking online, add it to the 'note' option.
After your appointment, if you have not received your superbill with your invoice, simply email me at jaz@sdherbalmedicine.com and we’ll send you an email with a PDF superbill within a few business days.
For treatment plans or multi-session treatments, we will provide a superbill after the completion of the treatment plan or one per visit—whatever works best for your records.

A Quick Note on Insurance Responsibility
While we’re happy to provide a superbill for potential reimbursement, it is your responsibility to confirm what your specific insurance plan will cover. Every plan is different, and we are not able to verify benefits or guarantee reimbursement on your behalf.
